2010 ford fusion is vibrating really bad. Has 70,900 miles on it?

okay so here is the problem. My car is vibrating really bad when in idle and is in drive. If i put it into neutral its not that bad, if the ac/heater is off it still vibrates just not as bad. When I go to take off from a red light its vibrating really bad. Again if the ac/heater is off its not as bad but still very noticeable. You can tell when you go from neutral to drive even in revers you can tell the difference in all gears its vibrating and we looked at the motor mounts and they looked perfect. After you get in about 3rd gear its not vibrating at all. When taking off if it jumps to 3000 rpms its starts to vibrate if i start off slow and slowly start to move up in rpms it doesnt vibrate. Thats all i can think of right now if i think of anything else ill add more on to it
