2.0 tdi 2008 Ford Focus titanium facelift model limp mode?!

Hello there a few months ago My cars engine management light came on and it went into a limp mode ( nothing other 3k revs) and I took to a garage and it was a little rip in a pipe where my dpf is. Well I got that sorted but a few weeks after I started it up and gave it some gas but once I had slowed down it went into a limp mode again with no engine management light on. So I turned it of and on again and problem had gone. But it's been happening allot since where when I randomly give it some gas it going into a limp mode and I have to turn it off and on again. So does anyone have a clue what's going on?

My car is a Ford Focus 2008 2.0 tdi titanium and it's done 117k miles. Help would be much appreciated!
